Job Title: Physical Therapist (PT) SNF / LTCCentra Healthcare Allied is seeking a skilled Physical Therapist to join our team for a travel assignment in California.Job Summary:We are looking for a highly motivated and experienced Physical Therapist to work in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) setting. The ideal candidate will have a strong background in physical therapy and excellent communication skills.Responsibilities:

• Provide physical therapy services to patients in a SNF / LTC setting
• Develop and implement individualized treatment plans to meet patient needs
• Conduct thorough assessments and evaluations to identify patient goals and objectives
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Document patient progress and maintain accurate recordsRequirements:
  • 1 year of recent experience as a Physical Therapist in a SNF / LTC setting
